A "consent to release my medical records" form must be signed to issue the transfer or release of medical records. The form must be signed by the patient if they are 16+ , the parent/ guardian if they are 15 years old or younger, or by the power of attorney. The medical release form can be located in the Patient Resources section. There is a fee for the chart transfer.

To speak with a doctor over the phone you must first schedule an appointment. If you do not have an appointment the doctor will not be able to speak with you as they are occupied with other patients.
  • Are you accepting new patients for family medicine?
At the moment we are compiling a secondary waiting list for the family medicine practice here at Restore Medical Clinics. To be added to the list please fill out the Family Physician Waitlist registration form below. Please be aware this is not a guarantee that you will be registered with our clinic. In the future, as physicians accept new patient, we will contact the patients on the list in chronological order to set up a meet and greets with the future doctor.
  • My name is on the waitlist. When will I be assigned a family doctor?
We want to thank you for your understanding as we navigate this time. Our facility is under construction and not yet ready to accommodate family practice patients. We do not presently have an exact date for the opening of the family practice, but when our clinic is prepared to accommodate all of our family physicians and their new patients we will contact you.
  • What is an FAQ section?
An FAQ section can be used to quickly answer common questions about you or your business, such as “Where do you ship to?”, “What are your opening hours?” or “How can I book a service?” It’s a great way to help people navigate your site and can even boost your site’s SEO.
